Invitation to Participate: I, _____________, am invited to
participate in the abovementioned research study conducted by Sahashi
Takami, MBI Researcher.
Purpose of the Study: The purpose of the study is to observe
the effects of newly-developed medicines ab.n2f.zf0.0b 01-20 on an
adjusted human body.
Participation: My participation will consist essentially of
allowing physical actions to be taken upon my body, after which I will
ingest medicine and allow the researcher to observe the results.
Risks: My participation in this study will entail that I take
risks personal, emotional, psychological, physical, social, and
potentially economic. The researcher would like to assure you that
every effort will be made to minimize or at least contain these risks
by means of counteracting agents and antidotes and potentially the
adjusting of the flow of time itself. Death will not be permitted and
the researcher would also like to assure you that should risks be
unavoidable we will be offering complementary memory-wiping of all
testing to reduce mental trauma to the volunteer.
Benefits: My participation in this study will aid in the
survival of humanity, the recovery of previously-shutdown Sekirei, and
the development of Ashikabi healing prowess to assist in surviving the
S-plan's Stage 4 proceedings.
Confidentiality and anonymity:The information I will share will
remain strictly confidential to MBI Corporation. I understand that the
contents will be used only for SCIENCE and that my confidentiality
will be protected by removal of the memory of any or all witnesses or
of the witnesses themselves.
Conservation of data: The data collected (physical or digital)
will be kept in a secure manner by being placed inside a box hidden
inside another box placed in a safe carefully hidden behind
[classified] in [classified] which will be cleverly disguised in a
bathroom on the [classified] floor of MBI Corporate Head Office, which
is restricted access only to MBI Researcher Ichinomi Natsuo
and Sekirei 04 Karasuba. The bathroom itself is, to further assure
you, guarded by leopards. It will be kept until destroyed by the
passage of time or we break it.
Compensation: Participants in this study will receive 2500
points for their team.
Voluntary Participation: I am under no obligation to
participate and if I choose to participate, I can withdraw from the
study at any time and/or refuse to answer any questions, without
suffering any negative consequences due to the researcher's current
control of my, and my loved ones', physical wellbeing. If I choose to
withdraw, all data gathered until the time of withdrawal will be shut
in the aforementioned secure manner (see: Conservation of Data.)
Acceptance: I, __________________ , agree to participate in the
above research study conducted by Sahashi Takami of the Mid-Bio
Informatics Corporation, which research is under the supervision of
Hiroto Minaka, CEO.
